For Good Dog Teams Welcome to the site for Good Dog teams! This page was created especially for our volunteers to receive updates and announcements, share information, and access Good Dog resources. Please visit this site often for important news just for Good Dog teams! Order Your Official Good Dog Attire!Good Dog now has vests, bandanas and patches for sale exclusively to active, certified Good Dog teams. Print out the Certified Good Dog Attire Order Form, mail in your payment, and you'll receive your order in four to six weeks. As a reminder, all volunteers receive a complimentary bandana upon graduation. Replacements must be ordered separately. Good Dog attire is for current, certified Good Dog teams only. Good Dog Vet FormThe Good Dog Vet Form must be updated annually for Good Dogs to maintain their active status. When needed, please download and print the Vet Form from this website, have your vet fill it out, and send it in to the Good Dog office. Volunteer ProfileIf your information, such as address, phone, or email changes, please let us know via the Volunteer Profile Form. Volunteers can simply type in their information and send it directly to the Good Dog office from this website. It's a convenient way for you to keep us updated, so that we can keep in touch with you! Order Your Good Dog Trading CardsCertified and current teams may order their official Good Dog Trading Cards here from our partner Custom-TradingCards.com. Incident Report FormPlease fill out this incident report form if anything irregular, upsetting or against Good Dog and/or facility policy happens on an official Good Dog therapy dog visit. This includes issues with other volunteers, dogs, facility staff or clients. Please send it our office immediately to protect yourself and the organization.
